GMICS Network is building its links with GM Health and Social Care Partnership (GMHSCP), the Directors of Adult Services (GM DASS) group, with NHS England and other NHS partners.
The independent care sector is substantially the delivery arm of care for older people in the community through care homes and home care services. The bulk of this is commissioned care – local authorities and health bodies commission it on behalf of individuals with assessed needs – and the way providers and commissioners work together has to improve and more integrated approaches are possible solutions for this.
There is a significant fault line to work across between care home and home care providers and hospital wards. Reducing unnecessary admissions to hospital and making the experience of admission and discharge a better experience for the individual is also something better integration of services can help deliver.
One mechanism in place for better joint working is the workstreams in place. These are multi-agency forums for focusing on specific areas of change required. GMICS Network is linked with the Residential and Nursing Care Delivery Group, the Living Well at Home group (includes domiciliary care), the Quality Forum exploring a GM-wide quality commitment and the Quality Improvement and Best Practice group.
